Have you fainted more than 1 time when trying to stand up?

Yes

Do you also have coronary artery disease, heart failure, or a history of an irregular heartbeat (arrhythmia)?

Yes

Do you also have high blood pressure or a history of stroke or transient ischemic attack (TIA)?

Yes

Do you continue to feel lightheaded after fainting?

Did you faint, but you are now fully alert without other symptoms?
